Escuela de Ciencias; Modelado MatemáticoIntroduction: Back pain, patellofemoral syndrome and "growing pains" are increasingly common in children and adolescents, both with incidence and precocity of appearance. In most cases their etiology is unknown. Objective: The objective of this research was to explore the possible association of these three entities with pelvic anteversion. Methods: This is a cross-sectional study with case-control analysis, comprising a group of 30 symptomatic children aged between 6 and 17 years (cases) who had consulted for low back pain, "growing pains" or sore knees, and a control group of 30 children with no history of pain in these areas (controls). To measure the position of the pelvis and other biomechanical variables a software programme was developed that quantifies these measurements from reflective markings located on specific anatomical landmarks of each child. Other measurements were obtained through semiological tests and interviews with children and their parents. Results: Children and adolescents with pain had more pelvic anteversion compared with controls (mean 13.3 and 5.4 degrees, p < 0.001). They also had less strength in muscles: Lumbar extensor, abdominals, psoas and diaphragm. We found that psoas is the muscle most responsible for pelvis anteversion in symptomatic group. Compared with children without pain, those with back pain, patellofemoral syndrome or "growing pains" have 4.2 times more frequent poor or regular strength in the column extensor muscles. They also have nearly 18 times as likely to have a pelvic angle greater than 10 degrees compared with no pain children. Conclusions: Children and adolescents who suffer from back pain, patellofemoral syndrome and "growing pains" have increased pelvic anteversion and lower muscle strength, mainly in the thoracolumbar spine extensors, abdominals, diaphragm and psoas. The study found that the psoas muscle is most responsible for the development of pelvis anteversion. However, selection of appropriate MBT device characteristics is critical for ensuring both their mechanical soundness and their optimal distraction function for each patient's condition. This article assesses six characteristics of currently available MBT devices to characterize their design and function and to classify them in a way that assists the selection of the best device option for each clinical case. To assess changes, measurements were taken at baseline and at one-year follow-up with the Beck Anxiety Inventory and the Illness Perception Questionnaire, brief version, taking into account the importance of perceptions in the process of adjusting to illness and acquiring healthy life habits. Hypotheses were tested by a structural model. The results obtained from this study showed that illness perceptions were related to anxiety levels at both time points; however, the relations were stronger before cognitive behavioral therapy (ßt0 = 0.61, p < 0.01; ßt1 = 0.37, p < 0.01). Cognitive behavioral therapy was found to be a moderator of changes in both illness perceptions and anxiety at the time of follow-up (ß = -0.31, p < 0.01; ß = -0.26, p < 0.01). The objective of this study was to estimate the cost-effectiveness of thermotherapy versus pentavalent antimonials for the treatment of cutaneous leishmaniasis. Methods: Effectiveness was the proportion of healing and safety with the adverse effects; these parameters were estimated from a controlled clinical trial and a meta-analysis. A standard costing was conducted. Average and incremental cost-effectiveness ratios were estimated. The uncertainty regarding effectiveness, safety, and costs was determined through sensitivity analyses. Results: The total costs were $66,807 with Glucantime and $14,079 with thermotherapy. The therapeutic effectiveness rates were 64.2% for thermotherapy and 85.1% for Glucantime. The average cost-effectiveness ratios ranged between $721 and $1275 for Glucantime and between $187 and $390 for thermotherapy. Based on the meta-analysis, thermotherapy may be a dominant strategy. Departamento de Ingeniería Mecánica; Bioingeniería GIB (CES – EAFIT)The aim of this study was to develop a three-dimensional finite element model of the mandible, including its TMJ. The model consisted of 7942 nodes and 41,010 elements, which were obtained from a convergence test, done to minimize the result error. It included cancellous and cortical bone, periodontal ligament, masticatory muscles (masseters, temporalis, lateral and internal pterygoids), teeth and the articular disk. All characteristics such as dental, mandibular, and muscle geometry were obtained from a computerized tomography (CT) of a living person. CT sections were scanned and digitized with a CAD software program. After images were adequately assembled, a vertical tracing was done which allowed the definition of a three-dimensional mesh. Modeling of teeth was carried out independently and the periodontal ligament was later included, limiting the alveolar area. Muscles were modeled based on flat-scale photographs and total muscle force was distributed in multiple vectors. The articular disk was generated having 2 mm of thickness with the combination of spring-type (axial stiffness) and gap-type (contact) elements. The model was then analyzed with finite element method (FEM) software where a mesh was generated and values for Poisson's ratio, elasticity, and shear modulus were assigned. These were orthotropic for cancellous and cortical bone, and isotropic for dentin, periodontal ligament, articular disk, and temporal bone. The boundary conditions were defined restricting the nodes on the periphery of the temporal bone. Design/methodology/approach: With this objective, 167 semi-structured interviews were conducted with millennials who work at 10 Colombian companies from the manufacturing and service sectors, located in the five main cities of the country. With a qualitative approach, in the interviews, the research team used a strategy inspired by the technique of generating visual structures associated with grounded theory. Findings: It is concluded that new generations of Colombian workers know of the importance of rewards and autonomy in work and are more critical and less passive in the face of unhealthy working conditions. At the same time, their conduct and speeches are the consequence of the characteristics of the Colombian labour market. The document responds to the need to deepen the debates on welfare and happiness in organizations and to include the demands of millennials in the reflective and political horizon of the ideas of healthy employment and decent work. In practice, this article seeks to demystify ideas about millennials in Colombia and critically contribute to reflection on intergenerational relations in organizations and salary and welfare models. As a Latin American case, it is an original contribution that avoids the common places and the frivolity with which the insertion of the new generations into the working world has been analysed. Practical implications: In practice, this paper seeks to demystify ideas about millennials in Colombia and critically contribute to reflection on intergenerational relations in organizations and salary and welfare models. Departamento de Ingeniería de Producción; Materiales de IngenieríaObjective: This study aimed to identify the changes in the time-dependent deformation response of coronal dentin with ageing and its relationship with changes in chemical composition. Background: The structural behaviour of dentin with ageing is affected by changes in the density and diameter of its dentinal tubules (ie porosity), as well as changes in chemical composition throughout the tooth. However, little is known about the time-dependent deformation behaviour of aged dentin and the importance of its hierarchical structure and variations in chemical composition. Materials and methods: The spherical indentation response of aged coronal dentin was analysed in the outer, middle and inner regions, and its time-dependent deformation response was modelled in terms of its microstructure and chemical composition using a model recently proposed for young dentin. Results: The viscous deformation behaviour of aged dentin followed a power-law response with a decrease in the stress exponent when compared to young dentin. These results can be explained by cross-linking of the collagen present in the tissue. Conclusion: A decrease in the deformation ability of aged dentin was found. This behaviour could be a result of a dissolution process and reprecipitation of the minerals present in intertubular dentin into the dentinal tubules. © 2018 John Wiley & Sons A/S and The Gerodontology Association. Departamento de Ciencias; Lógica y ComputaciónDeterministic SIR models were applied to simulate Susceptible-Infected-Removed and to estimate the threshold condition for varicella outbreaks in children, reported in Medellin, Colombia. The expected numbers of susceptible, infected and removed individuals were compared with observed cases from notification of varicella outbreaks to the local Board of Health and from survey data. The threshold condition was estimated by the basic reproductive ratio and by the relative removal rate, through which measures for preventing and curtailing the outbreaks were identified. The model demonstrated a reasonable fit to the observations, except in two of the six outbreaks which probably reflected under-registration of cases. In order to have prevented these outbreaks, between 4.4% and 52.9% of the susceptible population should have been vaccinated assuming an 85% vaccine effectiveness. Escuela de Ciencias; Modelado MatemáticoThis article investigates drug dosage individualization when the patient population can be described with a random-effects linear model of a continuous pharmacokinetic or pharmacodynamic response. Specifically, we show through both decision-theoretic arguments and simulations that a published clinical algorithm may produce better individualized dosages than some traditional methods of therapeutic drug monitoring. Since empirical evidence suggests that the linear model may adequately describe drugs and patient populations, and linear models are easier to handle than the nonlinear models traditionally used in population pharmacokinetics, our results highlight the potential applicability of linear mixed models to dosage computations and personalized medicine. Departamento de Ciencias; Modelado MatemáticoPressure Injuries are chronic wounds that are formed due to the constriction of the soft tissues against bone prominences. In order to assess these injuries, the medical personnel carry out the evaluation and diagnosis using visual methods and manual measurements, which can be inaccurate and may generate discomfort in the patients. By using segmentation techniques, the Pressure Injuries can be extracted from an image and accurately parameterized, leading to a correct diagnosis. In general, these techniques are based on the solution of differential equations and the involved numerical methods are demanding in terms of computational resources. In previous work, we proposed a technique developed using toroidal parametric equations for image decomposition and segmentation without solving differential equations. In this paper, we present the development of a mobile application useful for the non-contact assessment of Pressure Injuries based on the toroidal decomposition from images. The usage of this technique allows us to achieve an accurate segmentation almost 8 times faster than Active Contours without Edges (ACWE) and Dynamic Contours methods.We describe the techniques and the implementation for Android devices using Python and Kivy. Departamento de Ciencias; Modelado MatemáticoThe understanding of a psychological phenomena such as emotion is of paramount importance for psychologists, since it allows to recognize a pathology and to prescribe a due treatment for a patient. While approaching this problem, mathematicians and computational science engineers have proposed different unimodal techniques for emotion recognition from voice, electroencephalography, facial expression, and physiological data. It is also well known that identifying emotions is a multimodal process. The main goal in this work is to train a computer to do so. In this paper we will present our first approach to a multimodal emotion recognition via data fusion of Electroencephalography and facial expressions. The selected strategy was a feature-level fusion of both Electroencephalography and facial microexpressions, and the classification schemes used were a neural network model and a random forest classifier. Experimental set up was out with the balanced multimodal database MAHNOB-HCI. Results are promising compared to results from other authors with a 97% of accuracy. The feature-level fusion approach used in this work improves our unimodal techniques up to 12% per emotion. Finite elements analysis of three and four implant-supported prostheses was performed to determine the stresses in the superstructure, implants and cortical bone and, therefore, the failure prediction for each restoration. METHODS: An edentulous mandible was modeled from CT scan images. Two finite element models of three and four implant-supported prostheses with cantilever lengths of 10 and 15 mm were created. Occlusal loads in different parts of the superstructure were applied and shear and normal stresses were calculated. RESULTS: Two failure criteria were analyzed: the von Mises criterion for isotropic materials (superstructure and implants) and the Tsai-Wu criterion for transversely isotropic material (cortical bone). Both criteria predict failure in the three implant-supported prosthesis for all cases analyzed. The same applies for the four-implant prosthesis of 15 mm cantilever length. However, four implants and a cantilever length of 10mm passed the failure criteria and were considered safe. CONCLUSIONS: The results from the patient analyzed showed that fixed support prostheses on three implants are not recommended from a structural point of view because they do not adequately support occlusal loads. Excessive stress in the superstructure and the cortical bone can be expected, which would anticipate the failure of the restoration. Departamento de Ingeniería de Diseño; Ingeniería de Diseño (GRID)Cyber-physical systems (CPSs) have sophisticated control mechanisms that help achieve optimal system operations and services. These mechanisms, imply considering multiple signal inputs in parallel, to timely respond to varying working conditions. Despite the advantages that control mechanisms convey, they bring new challenges in terms of failure prevention. The compensatory action the control exerts cause a fault masking effect, hampering fault diagnosis. Likewise, the multiple information inputs CPSs have to process can affect the timely system response to faults. This article proposes a failure prognosis method, which combines time series-based forecasting methods with statistically based classification techniques in order to investigate system degradation and failure forming on system levels. This method utilizes a new approach based on the concept of the system operation mode (SOM) that offers a novel perspective for health management that allows monitoring the system behavior, through the frequency and duration of SOMs. Validation of this method was conducted by systematically injecting faults in a cyber-physical greenhouse testbed. The obtained results demonstrate that the degradation and fault forming process can be monitored by analyzing the changes of the frequency and duration of SOMs. These indicators made possible to estimate the time to failure caused by various failures in the conducted experiments. © 2020 by the authors. While there have been a number of studies aimed at understanding the hardness and crack growth resistance behavior of this tissue, no study has evaluated if cracks in this tissue undergo repair. In this investigation the crack repair characteristics of young human enamel were evaluated as a function of patient gender and as a function of the distance from the Dentin Enamel Junction (DEJ). Cracks were introduced via microindentation along the prism direction and evaluated as a function of time after the indentation. Microscopic observations indicated that the repair of cracks began immediately after crack initiation and reaches saturation after approximately 48 h. During this process he crack length decreased up to 10% of the initial length, and the largest degree of reduction occurred in the deep enamel, nearest the DEJ. However, it tends to remain inconsistent, infrequent or non-existent, as does its acceptance by intensive care physicians. This study sought to explore the perceptions, level of knowledge, perceived barriers, and practices of physicians regarding palliative care practices (PC) in Intensive Care Units (ICU). Methods: Descriptive-correlational study. Participating physicians working in ICU in Colombia (n = 101) completed an ad hoc questionnaire that included subscales of perceptions, knowledge, perceived barriers, and PC practices in ICU. A Structural Equation Model (PLS-SEM) was used to examine the reciprocal relationships between the measured variables and those that could predict interaction practices between the 2 specialties. Results: First, results from the measurement model to examine the validity and reliability of the latent variables found (PC training, favorable perceptions about PC, institutional barriers, and ICU-PC interaction practices) and their indicators were obtained. Second, the structural model found that, a greater number of hours of PC training, a favorable perception of PC and a lower perception of institutional barriers are related to greater interaction between PC and ICU, particularly when emotional or family problems are detected. Conclusions: PC-ICU interactions are influenced by training, a positive perception of PC and less perceived institutional barriers.
